Fire Safety Plan

As part of the application process we also require a complete, detailed fire safety plan from all performers and installation artists who intend to work with open flame. Even if we have worked with you in the past, you must still fill out this years fire safety plan in its entirety in order to be considered for this year's festival. Please note that the information you provide here will be included in the fire safety plan for the entire event. Our acceptance of your proposal depends on your taking this questionnaire seriously and answering all applicable questions as though you are addressing the OFD Fire Inspector (because you are).

Name of Installation/Performance
1. Please provide an overview of the flame effect components of your installation or performance. If you are a performance artist, please list the types and numbers of fire tools you intend to use in your performance. Please also describe any “exotic” fire tools that you use, especially ones that The Crucible might not have encountered before. (This should be a general but comprehensive description; detailed questions follow).
2. What type(s) of fuel will you be using and in what quantities? Specifically, if you are an installation artist, tell us how much fuel (of each type) your installation uses per hour. If you are a performance artist, tell us how much fuel (of each type) is used by each fire tool you intend to use in your performance.
3. Does your system use forced air or oxidizers other than atmospheric air? If so, please describe the oxidation system and the volumes of oxidizers consumed per unit time by the system.
4. If you are introducing any other materials (chemicals, metal powders, etc.) to your flames, please list the materials and the quantities you intend to use.
5. Please provide a detailed description of the structure and operation of your installation, or of any fire performance tools whose flame effects are more complicated than fuel-soaked wicks. Include descriptions of fuel storage or holding vessels, fuel delivery mechanisms, manual or remotely-controlled valves, expansion or accumulator tanks, forced air blowers, manifolds, ignition systems, details of the flame head(s) (e.g., do they include venturis, etc.), and your fire-control system. Please send a labeled schematic showing all major components via email at FAF@thecrucible.org.
6. Describe the locations and positions of each of your system’s flame heads (e.g., how high off the ground, how close to one another); their shapes, sizes and orientations; and the directions, dimensions and durations of the flames they produce. If there are variations in the size and intensity of the flames emitted from any of the flame heads, please describe: • the minimum and maximum effects in terms of size and intensity; • the duration of the transition from minimum to maximum; • the average and maximum duration of the maximal effect; • the duration of the transition back to mimimum; • how the transitions are controlled. Please provide a ground-plan of your installation if it involves multiple flame appliances. (Email to FAF@thecrucible.org).
7. What is the minimum safe distance required for audience members from your installation/ performance?

9. List any flammable items that will be in close proximity to your installation/performance: (e.g.: wood, paper, plastic, synthetic clothing etc.)
10. Describe your normal, orderly shutdown procedure. That is, what you do at the end of a performance to extinguish your flames, purge your lines, etc., to ensure that your installation or fire tools can be left safely unattended.
11. Describe your emergency shutdown procedure. That is, what event fire safety personnel need to know to shut down your installation or performance tools in case you and/or your support personnel are incapacitated.

12b. If you need additional loaner fire extinguishers beyond those you have listed above, indicate how many of each type (CO2, H2O, dry chem ABC) and size (5lb, 10lb, 15lb, etc.) you would like. (The Crucible will try to accommodate all reasonable requests, but cannot guarantee that you will receive all the extinguishers you request. Bringing as many of your own fire extinguishers as you can is the best way of ensuring that you will have all you need.)
